Social Worker Coordinator

Location:  Lake Charles, LA

Summary:

The Social Worker Coordinator provides coordination, education and outreach services to Medicaid members in the Louisiana Medicaid program. The Social Worker Coordinator supports the delivery of care coordination/case management services by removing barriers to the process if they fall within the realm of social work and enlists community resources. Primary responsibilities include assessing social service needs, identifying and addressing social barriers to members achieving optimal health care status, such as lack or inaccessibility of housing, transportation, utilities, or food. Additional responsibilities include assisting providers in accessing medically necessary specialty care services and ensuring members access appropriate preventive services.

Essential Duties and Responsibilities:

  • Support Medicaid member Care Coordination activities by following up with members referred by Nurse Care Coordinators or Medical Home Providers for assistance, particularly in relation to accessing specialty care or preventive services
  • Update state and community resource lists for Care Management Department
  • Provides information to members and providers regarding mental health and substance abuse benefits and community treatment resources
  • Works to develop contacts and enhance relationships with community agencies    
  • Serves as a member advocate providing support, triage and resolution of member issues
  • Serves as a facilitator to providers for resolution or provider concerns, complaints and grievances
  • Maintains and ensures the confidentiality of all information collected during the care coordination and utilization review process
  • Participates in developmental and quality training programs
  • Maintains appropriate level of experience and/or education to oversee care for specialty populations

Qualifications:

  • A minimum of three (3) years of Managed Care (i.e., Utilization Management or Case Management) experience is strongly preferred
  • Knowledge of chronic illnesses, co-morbidities and disease management
  • Knowledge of community based resources for the Medicaid population
  • Knowledge of adult learning and behavior change principles preferred
  • Clinical assessment skills, within state licensure scope of practice requirements
  • Crisis intervention skills, within state licensure scope of practice requirements
  • Ability to assess patient health status, within state licensure scope of practice requirements

Education and/or Experience:

  • Master’s degree in Social Work 
  • Minimum 3 years post-graduate clinical experience required

Certificates, Licenses, Registrations:

Licensed Clinical Social Worker (LCSW) in the state of Louisiana

Computer Skills:

Proficiency using software programs such as MS Word, ACCESS, PowerPoint, Excel and Outlook. Internet/web navigation skills necessary. Ability to type care coordination documentation directly into the system.

Environmental Factors/Physical Demands:

Work with the members in various hospitals, provider offices, and, in certain instances of high acuity, a member’s home to assess continuing needs. While performing the duties of this job, the employee is regularly required to have the ability to maintain active customer and employee communication; access, input and retrieve information from the computer system; enter alpha-numeric data into a computerized system often while listening on the telephone. May be subject to repetitive motion such as typing, data entry and vision to monitor. May be subject to bending, reaching, kneeling, stooping and lifting up to thirty (30) pounds.
